    Tradern: a collaborative model for improving small business participation in electronic commerce in sub-Saharan Africa.

    The inception of the Internet has brought with it Electronic Commerce (E-Commerce) practices which have greatly transformed the ways firms conduct businesses globally. Internet-based E-Commerce, particularly business-to-business (B2B) holds the key for small businesses to compete on a level playing field with their big business counterparts. Unfortunately, however, there is considerable evidence to show that small and medium sized enterprises (SMEs) in developing countries, particularly those in Sub-Saharan Africa (SSA), have not been reaping the benefits of this new commerce opportunity as their counterparts in North America and Europe. This chasm has given rise to another form of Digital Divide. This research has identified the major factors responsible for this state of affairs as the low level of participation by the SMEs in the SSA region in this global E-Commerce trade. SSA region has been identified as a region with the lowest level of economic, technological and Internet development in the world. There are 49 Countries in this region inhabited by over 633 million people representing about 10% of the world's population. They are characterised by a low income, low-levels of human resource development, as well as severe structural, social, political and economic weaknesses. All these have combined to make the region the poorest region in the world. It has been established that SMEs form the bedrock of every economy. There is also considerable evidence to suggest that the introduction of new technologies into organisations of all kinds and sizes has a major impact on the structure and functioning of the organizations. Furthermore, it has been established that B2B E-Commerce is contributing more to the global economy than all other forms of E-Commerce transactions. Equipping SMEs in developing countries, particularly those in SSA, by with emerging B2B technologies could help improve their growth potential which will invariably place them in a better position to contribute to the region's economic advancement. Existing models have failed in attracting large numbers of SMEs in the region, partly due to the prohibitive costs of these technologies which make them unaffordable by the poor SMEs. This research has identified that the issues of Trust as well as the problems associated with Fraud and Security also play a part in making E-Commerce unattractive to a lot of SMEs in SSA. Current theoretical frameworks have been extended by developing a new taxonomy showing the various components of E-Business where the distinctions between E-Commerce, E-Government, Tele-Medicine and E-Learning are clearly identified. This thesis seeks to find solutions to the identified problems by finding ways of attracting more SMEs in SSA to participate in the global E-Commerce endeavour. This is with a view to enabling them to leverage and maximise their E-Commerce potential, which in turn would help them exploit today's global E-Commerce opportunities. This will ultimately help them in contributing to the economic growth of the region. As a way of making E-Commerce attractive, affordable and profitable, an architectural model has been designed which, it is believed, would make the deployment and implementation of B2B E-Commerce more achievable for the poor SMEs in SSA. This model, known as Tradern Model, combines the trado-cultural and modern methods of conducting business. This combination would help SMEs deploying the technology to conduct their businesses using both methods without compromising their growth. Over and above everything else there is the benefit of a level-playing field which the SMEs in developing economies, like Africa, and particularly Sub-Saharan Africa, can capitalise on to leapfrog, improve and sustain their economic development and global e-business participation as a step towards bridging the digital divide

    General practitioners' knowledge, attitudes and views of providing preconception care: a qualitative investigation

    Background: Preconception health and care aims to reduce parental risk factors before pregnancy through health promotion and intervention. Little is known about the preconception interventions that general practitioners (GPs) provide. The aim of this study was to examine GPs’ knowledge, attitudes, and views towards preconception health and care in the general practice setting. Methods: As part of a large mixed-methods study to explore preconception care in England, we surveyed 1,173 women attending maternity units and GP services in London and interviewed women and health professionals. Seven GPs were interviewed, and the framework analysis method was used to analyse the data. Findings: Seven themes emerged from the data: Knowledge of preconception guidelines; Content of preconception advice; Who should deliver preconception care?; Targeting provision of preconception care; Preconception health for men; Barriers to providing preconception care; and Ways of improving preconception care. A lack of knowledge and demand for preconception care was found, and although reaching women before they are pregnant was seen as important it was not a responsibility that could be adequately met by GPs. Specialist preconception services were not provided within GP surgeries, and care was mainly targeted at women with medical conditions. GPs described diverse patient groups with very different health needs. Conclusion: Implementation of preconception policy and guidelines is required to engage women and men and to develop proactive delivery of care with the potential to improve pregnancy and neonatal outcomes.     Energy demands have been increasing due to the rise in population, various technological developments, and modern industrialisation. In order for countries to be sustainably developed, economic growth and constant energy production are key factors (Akyürek, 2018). As a key component in development of countries, various methods have been adopted to create cleaner energy using lowcost alternatives. Solid waste disposal, on the other hand, is presently a persistent problem facing most economies because of the rising output of waste. The speed at which waste is currently generated is greatly influenced by population, economy, and a rise in standards of living. Towns assigned with the purpose of managing waste have the challenge of making available an effective and sufficient system to carry out this function (Olukanni & Olatunji, 2018; Abarca et al., 2013; Olukanni et al., 2018; Olukanni et al., 2020). Agricultural waste, a type of solid waste which includes crops straw, husks, and livestock manure, and domestic waste have not been properly disposed of over the years, which has led to significant side effects. When these wastes are not properly disposed of, large numbers of chemical substances such produced, as nitrous and this oxide seriously (N2O), affects sulphur the dioxide environment (SO2), methane on a large (CH scale.

    Biogas Recovery from Poultry and Piggery Waste

    Fossil fuel, the current major source of energy, meets 88% of global energy demands. Its negative effect on the atmosphere through the release of greenhouse gases has led to an all-out search for alternative clean energy sources. This study focuses on exploring the potential recovery of biogas from poultry and piggery wastes and investigates improvements made in its production involving different digestion techniques. The study analysed the biogas production procedures of poultry and piggery waste from 2010 to 2020. Thirty works on recovery of energy from these organic wastes were carefully studied with specific objectives: extract the improvements made through co-digestion with various substrates, explore the different anaerobic digestion methods and possible results in the literature, and examine the different models adopted in the literature and the factors that affect the model sensitivity. It was observed after analysis that substrates co-digested alongside poultry and piggery waste and the models used greatly influenced the amount of biogas released. The study answered the questions regarding various other factors influencing production of biogas with temperature values of 25–50°C, pH used (6.3–8.7), hydraulic retention time (25–350 days) and total solids concentration (2–90%) across the literature reviewed. The produced biogas in the majority of the literature was improved by 11.04–28.78%. This work has contributed in no small measure to the efficacy of biogas production from poultry and piggery waste and will contribute to United Nations Sustainable Development Goals, with specific emphasis on goals 7, 12, and 13, which will boost the drive of national governments on sustainability

    Psychometric Development of the Research and Knowledge Scale

    BACKGROUND: Many research participants are misinformed about research terms, procedures, and goals; however, no validated instruments exist to assess individual\u27s comprehension of health-related research information. We propose research literacy as a concept that incorporates understanding about the purpose and nature of research. OBJECTIVES: We developed the Research and Knowledge Scale (RaKS) to measure research literacy in a culturally, literacy-sensitive manner. We describe its development and psychometric properties. RESEARCH DESIGN: Qualitative methods were used to assess perspectives of research participants and researchers. Literature and informed consent reviews were conducted to develop initial items. These data were used to develop initial domains and items of the RaKS, and expert panel reviews and cognitive pretesting were done to refine the scale. We conducted psychometric analyses to evaluate the scale. SUBJECTS: The cross-sectional survey was administered to a purposive community-based sample (n=430) using a Web-based data collection system and paper. MEASURES: We did classic theory testing on individual items and assessed test-retest reliability and Kuder-Richardson-20 for internal consistency. We conducted exploratory factor analysis and analysis of variance to assess differences in mean research literacy scores in sociodemographic subgroups. RESULTS: The RaKS is comprised of 16 items, with a Kuder-Richardson-20 estimate of 0.81 and test-retest reliability 0.84. There were differences in mean scale scores by race/ethnicity, age, education, income, and health literacy (all P \u3c 0.01). CONCLUSIONS: This study provides preliminary evidence for the reliability and validity of the RaKS. This scale can be used to measure research participants\u27 understanding about health-related research processes and identify areas to improve informed decision-making about research participation

    How do women prepare for pregnancy? Preconception experiences of women attending antenatal services and views of health professionals

Methods - Cross-sectional questionnaire survey of pregnant women attending three maternity services in London about knowledge and uptake of preconception care; including a robust measure of pregnancy planning, and phone interviews with a range of health care professionals. Main results - We recruited 1173/1288 (90%) women, median age of 32 years. 73% had clearly planned their pregnancy, 24% were ambivalent and only 3% of pregnancies were unplanned. 51% of all women and 63% of those with a planned pregnancy took folic acid before pregnancy. 21% of all women reported smoking and 61% reported drinking alcohol in the 3 months before pregnancy; 48% of smokers and 41% of drinkers reduced or stopped before pregnancy. The 51% of all women who reported advice from a health professional before becoming pregnant were more likely to adopt healthier behaviours before pregnancy [adjusted odds ratios for greatest health professional input compared with none were 2.34 (95% confidence interval 1.54–3.54) for taking folic acid and 2.18 (95% CI 1.42–3.36) for adopting a healthier diet before pregnancy]. Interviews with 20 health professionals indicated low awareness of preconception health issues, missed opportunities and confusion about responsibility for delivery of preconception care. Significance of the findings - Despite a high level of pregnancy planning, awareness of preconception health among women and health professionals is low, and responsibility for providing preconception care is unclear. However, many women are motivated to adopt healthier behaviours in the preconception period, as indicated by halving of reported smoking rates in this study. The link between health professional input and healthy behaviour change before pregnancy is a new finding that should invigorate strategies to improve awareness and uptake of pre-pregnancy health care, and bring wider benefits for public health.Department of Healt

    Short-Term Effects of Screening for Cardiovascular Risk in the Deaf Community: A Pilot Study

    There is limited information on the risk of cardiovascular disease amongst the Deaf community. Given that the access of Deaf people to mainstream health promotion is likely to be hindered by language barriers, we were interested to assess the short-term impact of cardiovascular health promotion within this group. Using a pilot study we investigated changes in cardiovascular risk factors amongst Deaf people identified to be at high cardiovascular risk, who received standard health promotion by a medical team specializing in cardiovascular health promotion. The short-term impact of cardiovascular health promotion in this group did not reduce estimates of cardiovascular risk. The reasons for this are likely to relate to the design and delivery of health promotion to Deaf people, which deserves further study

    Why do women invest in pre-pregnancy health and care? A qualitative investigation with women attending maternity services

    Background Despite the importance attributed to good pre-pregnancy care and its potential to improve pregnancy and child health outcomes, relatively little is known about why women invest in pre-pregnancy health and care. We sought to gain insight into why women invested in pre-pregnancy health and care. Methods We carried out 20 qualitative in-depth interviews with pregnant or recently pregnant women who were drawn from a survey of antenatal clinic attendees in London, UK. Interviewees were purposively sampled to include high and low investors in pre-pregnancy health and care, with variation in age, partnership status, ethnicity and pre-existing medical conditions. Data analysis was conducted using the Framework method. Results We identified three groups in relation to pre-pregnancy health and care: 1) The “prepared” group, who had high levels of pregnancy planning and mostly positive attitudes to micronutrient supplementation outside of pregnancy, carried out pre-pregnancy activities such as taking folic acid and making changes to diet and lifestyle. 2) The “poor knowledge” group, who also had high levels of pregnancy planning, did not carry out pre-pregnancy activities and described themselves as having poor knowledge. Elsewhere in their interviews they expressed a strong dislike of micronutrient supplementation. 3) The “absent pre-pregnancy period” group, had the lowest levels of pregnancy planning and also expressed anti-supplement views. Even discussing the pre-pregnancy period with this group was difficult as responses to questions quickly shifted to focus on pregnancy itself. Knowledge of folic acid was poor in all groups. Conclusion Different pre-pregnancy care approaches are likely to be needed for each of the groups. Among the “prepared” group, who were proactive and receptive to health messages, greater availability of information and better response from health professionals could improve the range of pre-pregnancy activities carried out. Among the “poor knowledge” group, better response from health professionals might yield greater uptake of pre-pregnancy information. A different, general health strategy might be more appropriate for the “absent pre-pregnancy period” group.     Challenges of rapid migration to fully virtual education in the age of the Corona virus pandemic: experiences from across the world

    The social disruption caused by the sudden eruption of the Corona Virus pandemic has shaken the whole world, influencing all levels of education immensely. Notwithstanding there was a lack of preparedness for this global public health emergency which continues to affect all aspects of work and life. The problem is, naturally, multifaceted, fast evolving and complex, affecting everyone, threatening our well-being, the global economy, the environment and all societal and cultural norms and our everyday activities. In a recent UNESCO report it is noted that nearly a billion and a quarter (which is 67,7 % of the total number) of learners have been affected by the Corona Virus pandemic worldwide. The education sector at all levels has been one of the hardest hit sectors particularly as the academic/school year was in full swing. The impact of the pandemic is widespread, representing a health hazard worldwide. Being such, it profoundly affects society as a whole, and its members that are, in particular, i) individuals (the learners, their parents, educators, support staff), ii) schools, training organisations, pedagogical institutions and education systems, iii) quickly transformed policies, methods and pedagogies to serve the newly appeared needs of the latter. Lengthy developments of such scale usually take years of consultation, strategic planning and implementation. In addition to raising awareness across the population of the dangers of the virus transmission and instigating total lockdown, it has been necessary to develop mechanisms for continuing the delivery of education as well as demanding mechanisms for assuring the quality of the educational experience and educational results. There is often scepticism about securing quality standards in such a fast moving situation. Often in the recent past, the perception was that courses and degrees leading to an award are inferior if the course modules (and sometimes its assessment components) were wholly online. Over the last three decades most Higher Education institutions developed both considerable infrastructure and knowhow enabling distance mode delivery schools (Primary and Secondary) had hardly any necessary infrastructure nor adequate knowhow for enabling virtual education. In addition, community education and various training providers were mainly delivered face-to-face and that had to either stop altogether or rapidly convert materials, exercises and tests for online delivery and testing. A high degree of flexibility and commitment was demanded of all involved and particularly from the educators, who undertook to produce new educational materials in order to provide online support to pupils and students. Apart from the delivery mode of education, which is serving for certificated programmes, it is essential to ensure that learners’ needs are thoroughly and continuously addressed and are efficiently supported throughout the Coronavirus or any other future lockdown. The latter can be originated by various causes and reasons that vary in nature, such as natural or socioeconomical. Readiness, thus, in addition to preparedness, is the primary key question and solution when it comes to quality education for any lockdown. In most countries, the compulsory primary and secondary education sectors have been facing a more difficult challenge than that faced by Higher Education. The poor or in many cases non-existent technological infrastructure and low technological expertise of the teachers, instructors and parents, make the delivery of virtual education difficult or even impossible. The latter, coupled with phenomena such as social exclusion and digital divide where thousands of households do not have adequate access to broadband Internet, Wi-Fi infrastructure and personal computers hamper the promising and strenuous virtual solutions. The shockwaves of the sudden demands on all sectors of society and on individuals required rapid decisions and actions. We will not attempt to answer the question “Why was the world unprepared for the onslaught of the Coronavirus pandemic” but need to ascertain the level of preparedness and readiness particularly of the education sector, to effect the required rapid transition. We aimed to identify the challenges, and problems faced by the educators and their institutions. Through first-hand experiences we also identify best practices and solutions reached. Thus we constructed a questionnaire to gather our own responses but also experiences from colleagues and members of our environment, family, friends, and colleagues. This paper reports the first-hand experiences and knowledge of 33 co-authors from 27 institutions and from 13 different countries from Europe, Asia, and Africa. The communication technologies and development platforms used are identified; the challenges faced as well as solutions and best practices are reported. The findings are consolidated into the four areas explored i.e. Development Platforms, Communications Technologies, Challenges/Problems and Solutions/Best Practices. The conclusion summarises the findings into emerging themes and similarities.     Maternal colonization with Streptococcus agalactiae and associated stillbirth and neonatal disease in coastal Kenya

    Streptococcus agalactiae (group B streptococcus, GBS) causes neonatal disease and stillbirth, but its burden in sub-Saharan Africa is uncertain. We assessed maternal recto-vaginal GBS colonization (7,967 women), stillbirth and neonatal disease. Whole-genome sequencing was used to determine serotypes, sequence types and phylogeny. We found low maternal GBS colonization prevalence (934/7,967, 12%), but comparatively high incidence of GBS-associated stillbirth and early onset neonatal disease (EOD) in hospital (0.91 (0.25-2.3)/1,000 births and 0.76 (0.25-1.77)/1,000 live births, respectively). However, using a population denominator, EOD incidence was considerably reduced (0.13 (0.07-0.21)/1,000 live births). Treated cases of EOD had very high case fatality (17/36, 47%), especially within 24 h of birth, making under-ascertainment of community-born cases highly likely, both here and in similar facility-based studies. Maternal GBS colonization was less common in women with low socio-economic status, HIV infection and undernutrition, but when GBS-colonized, they were more probably colonized by the most virulent clone, CC17. CC17 accounted for 267/915 (29%) of maternal colonizing (265/267 (99%) serotype III; 2/267 (0.7%) serotype IV) and 51/73 (70%) of neonatal disease cases (all serotype III). Trivalent (Ia/II/III) and pentavalent (Ia/Ib/II/III/V) vaccines would cover 71/73 (97%) and 72/73 (99%) of disease-causing serotypes, respectively. Serotype IV should be considered for inclusion, with evidence of capsular switching in CC17 strains
